Process to Progress

  • It all begins with taking the first steps of reaching out for care by completing our intake documents.

    This can be done by clicking on the button “Schedule an Appointment” or clicking here

    Our intake forms provide us with important information to understand your/your loved one’s needs and allow you to complete the other required documents needed for us to treat you or your loved ones.

    Once you complete our intake forms, our team will review your information and will reach out to you by email from our main email address, info@renewedcommunity.org, or by phone to schedule your initial evaluation. Please be sure to check your spam or junk folder if you haven’t received our email by 72 hours.

    If you will be using your health insurance to receive treatment, our team will verify your insurance benefits within 24-72 hours.

  • After completing your initial intake documents, our team will review your paperwork to ensure we are the right fit.

    We will then reach out to you by email from info@renewedcommunity.org or by phone to schedule your initial appointment.

    Once we schedule you initial appointment, we will ask you to complete other documents to help our providers understand what services you may benefit from the most.

  • Prior to your initial appointment, we will be sending you additional form(s) to assist our providers to get to know you and ensure all legal paperwork has been completed.

    If using insurance, we will be asking you to complete your profile with Headway and complete any additional form(s) needed to use your insurance.

  • During our initial visit, we will focus on getting to know you or your loved one as a person along with the struggles encountered. In this visit we will also determine if we are a good fit for your/your loved one’s needs. Your initial visit may last up to 40-60 minutes and may take 1-2 visits to complete.

  • Dedicated follow-up visits are the most important component of your process to progress. Depending on your needs and progress, you will typically meet with your provider every week to every 1 - 4 weeks.

  • Throughout your journey, we will focus on your treatment plan and adjust accordingly. We will continue to ensure your care works for you.
